If you are visiting our practice for the first time we ask that you complete our patient medical history questionnaire. Providing us with this information is helpful for when it comes to understanding your current situation and planning for the care that you will receive within our practice.

This questionnaire also has a space for you to list any concerns that you have about your oral health and any worries you have about visiting the Dentist.

Individual Assessment

Your individual assessment with one of our Dentists will include a detailed examination of your teeth and oral tissues, diagnostic x-rays and in some cases study models of your teeth.
